Where the hell is the Ireland 2011 Cashback form? All I've found so far is the UK form, where last year VAT recovery was processed in Dublin - so where's our form?!

This is very poor from Sony - to advertise in stores that you can receive up to €180 in cash back, and then turn around and make it nearly impossible to claim - even the store clerk in Curry's didn't know where to find the form when I asked him to show me where on the Sony website it was!
